Very little goes wrong inside a single screen. It goes wrong when information moves - patient to clinician, clinician to system, system back to patient - and those seams are where the design and the test coverage both belong.
Access is a feature
Low bandwidth, older devices, shared phones and interrupted sessions are the normal conditions, not the edge. A tool that assumes a good connection excludes the people who need it most, and it fails silently rather than loudly.
The audit trail is the product
Who saw what, who changed it and when is not reporting that gets added later - it is a structural decision made on the first day, in the data model. Retrofitting it means rewriting the history you do not have.
